1. Complete HYSA Volunteer Disclosure & Membership Form (revised 7/08*)
2. Color copy of State issued ID
3. One (1) color photo (1”x1”) (headshots only, no hats, sunglasses or funny faces)
4. $5 (payable to HYSA)
5. Signed member pass
6. Self-addressed stamped envelope
*For Oahu applicants only: Envelope not needed if picking up pass on 8/24.
For Oahu Coaches and Administrators:
Saturday, July 26 from 10:00 a.m. to 2:00 p.m.: Bring the above items to Manana Community Park (Pearl City) in lieu of mailing.
Sunday, August 24, from 10:00 a.m. to 2:00 p.m.: Pick up your completed passes at Manana Community Park (7/26 drop off only)

Kai Going to China!
Congratulations to Hawaii's own Natasha Kai, on her selection to the US Women's Olympic Team that will travel to China. Natasha is currently leading the US Women's National Team in goal scoring. Good Job Natasha!
