HYSA e-Travel Forms
- Log in to your Affinity Soccer Account to process Travel Notifications and Rosters
- Approved Roster is automatically sent to the email on record. Make sure to request player cards from your league/club registrar if necessary. Travel permission is not required for travel within the State of Hawaii. Medical release forms should be obtained prior to any travel.
